1. Eligibility.
The drawings are open only to legal residents of the United States who are eighteen (18) years of age or older. Only individuals complete the survey by May 22, 2023 at 11:59pm PT are eligible to win. Eligibility to participate in the drawings is void wherever prohibited or restricted by law. Duplicate or more entries are disqualified and voided.

2. No Purchase Necessary
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THE DRAWING.

3. Entry.
To enter the drawings, you must complete the survey by May 22, 2023 at 11:59pm PT and supply your full name, address, phone number and email.

4. Odds.
The odds of winning depend on the number of surveys completed.

5. Drawings.
The drawings will take place at Crohn’s & Colitis Foundation, on the advertised date, and the winners will be announced to the audience via Social. The winners will be selected by or under the supervision of Crohn’s & Colitis Foundation, in its sole discretion, in random drawings from all eligible entries, which include one (1) entry per eligible survey made prior to the time of each drawing.

6. Prizes.
Expenses not specifically provided for in these Official Rules are the sole responsibility of the winners. All federal, state and local taxes and duties are the sole responsibility of the winners. The prizes may not be transferred, substituted or redeemed for cash, except that Crohn’s & Colitis Foundation may substitute a prize of equal or greater value due to prize availability.

7. Delivery.
If you are selected as a winner, Crohn’s & Colitis Foundation will contact you at the email address you provided with your survey, and you will need to provide an address where the prize may be delivered. Crohn’s & Colitis Foundation may retain a third party to handle prize delivery and you agree that Crohn’s & Colitis Foundation may provide them with your contact information. Crohn’s & Colitis Foundation is not responsible for missing,  damaged, broken, lost, stolen, or misdirected prizes.

8. Release of Name; Affidavit of Eligibility.
By entering the drawings, you agree that if you are selected as a winner Crohn’s & Colitis Foundation may release your name, upon request to participants or as otherwise legally required. A winner may be required to sign and return an affidavit of eligibility, or the prize may be forfeited. The winners may be requested to sign a publicity release; however, execution of a publicity release is not required to accept a prize.

9. Modification of Drawing.
Crohn’s & Colitis Foundation reserves the right in its sole discretion to cancel or modify the drawings if the drawings’ integrity is compromised in any way, and it may select winners from eligible non-suspect entrants. All federal, state and local laws and regulations apply.

10. Bound by Rules.
By entering the drawings, you fully and unconditionally agree to be bound by these Official Rules and the decisions of Crohn’s & Colitis Foundation, which will be final and binding in all matters relating to the drawings.  Failure to comply with these Official Rules may result in disqualification from the drawings. Crohn’s & Colitis Foundation reserves the right to verify the eligibility of entrants and to disqualify permanently from the drawings any entrant it believes has violated these Official Rules.

11. Use of Data.
Crohn’s & Colitis Foundation will collect personal data about entrants in accordance with its privacy policy. Please review Crohn’s & Colitis Foundation’s privacy policy here. By participating in the drawings, you agree to Crohn’s & Colitis Foundation’s collection and usage of your personal information and you acknowledge that you have read and accepted Crohn’s & Colitis Foundation’s privacy policy.

12. Release by Entrants.
By participating in the drawings, you agree to release, discharge, and hold harmless Crohn’s & Colitis Foundation, and its parents, affiliates, subsidiaries, advertising and promotional agencies and their respective members, officers, directors, employees and agents, from any liability, claims, losses, and damages arising out of or relating to these prize drawings, including without limitation your participation in Crohn’s & Colitis Foundation’s drawing.

13. Release by Winner.
By accepting a prize, winner agrees that Crohn’s & Colitis Foundation and its affiliates, subsidiaries, advertising and promotional agencies and their respective members, officers, directors, employees and agents will have no liability or responsibility for, and they will be held harmless against, any injuries, damages or losses of any kind to any person (including death) or property resulting in whole or in part, directly or indirectly, from the entry, receipt, acceptance, possession, use or misuse of the prize. If a winner executes a publicity release, the winner agrees, where lawful, that Crohn’s & Colitis Foundation may use the winner’s name, likeness, biographical information, voice, and statements attributed to winner (if true) for publication, advertising and promotional purposes, without consideration.

14. Choice of Law and Venue; Arbitration.
All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrants and Crohn’s & Colitis Foundation in connection with the drawings, shall be governed by, and construed in accordance with, the laws of the State of New York, without regard to its conflict of law provisions, except as to matters relating to arbitration, which shall be governed by the Federal Arbitration Act. Any dispute arising out of or relating to the drawings or any prize will be decided only by arbitration, individually and not on a class-wide basis. Any arbitration will be administered by the American Arbitration Association and will take place in New York, New York. Any and all claims or awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering the drawings, but in no event attorney’s fees; and under no circumstances will an entrant be permitted to obtain awards for, and you hereby waive all rights to claim, punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATION OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU.
